Intermediate Care Team
The Intermediate Care Service provides an alternative to hospital care for patients who still need some short-term help.
City Community Intermediate Care is an alternative to hospital care for patients who are able to leave hospital early, but who still need some short-term help, for example after a fall or following an illness or surgery, or who require a social care package and are awaiting assessment. Patients must be over 18 years of age and registered with a GP in Stoke on Trent.
The team consists of Qualified Nurses, Healthcare Support Workers, Rehabilitation Support Workers, Occupational Therapists, Physiotherapists, Social Workers, Mental Health Practitioner and Age Concern staff.
Community Intermediate Care also offers a bed based service at The Meadows, Berry Hill, St Michael's House, Chell and has 50 beds within a nursing home in Stoke on Trent.
Rehabilitation can also be arranged for those patients who are medically fit and prefer to stay in their own home.
