Heart Failure Service
The heart failure nurse service provides support to patients with a diagnosis of heart failure through regular assessment.
The heart failure nurse service provides nurse led clinics across Stoke on Trent and North Staffordshire for patients confirmed with a diagnosis of left ventricular systolic dysfunction (heart failure). They will also see patients in their own homes where appropriate.
Each patient is provided with the contact details of their heart failure nurse.
The team monitor patients on a regular basis depending on the severity of their condition and provide :-
• Drug management
• Psycho social support
• Ensure specialist involvement where appropriate
• Co-ordinate GP/cardiology input as required
• Provide palliative care assessments and treatments
• Phlebotomy ( taking blood)
• Organise specialist cardiac investigations as required
The service also has close working relations with the heart failure nursing team at University Hospital of North Staffordshire using a shared data base of information of patients which helps provide continuity of care and excellent communication between the two organisations.
