Falls Programme
The Falls Prevention Service is available for people who are aged 65 years or over and registered with a GP in Stoke on Trent

Mrs X broke her hip when she fell, after her treatment at hospital she had a short stay at The Meadows Rehabilitation Centre.The aim of the service is to identify older people who have had a fall or are at risk of having a fall which can result in a serious injury like broken bones or hip.
In order to identify patients at risk an assessment form is used which shows whether the patient is a low,medium or high risk of having a fall.
Patients or carers as well as healthcare professionals can complete the Assessment Form and send it to the Team at Burslem Health Centre.
The Stoke on Trent Integrated Falls Service works in conjunction with health professionals, Social Services and the voluntary sector and are based at:-
