Coronary Heart Disease Specialist Team

The Coronary Heart Disease service provides education, advice and support to people with Coronary Heart Disease (CHD). 

The Coronary Heart Disease (CHD) Specialist Nurse Team provides education, advice and support to patients with CHD, for example Myocardial Infarction (Heart Attack) and Angina

The team review patients with CHD mainly in a home setting following referral from hospital, their GP practice or other Health Professionals

The CHD Nurses provide patients/carers with a variety of information which includes some of the following:

•    Diagnosis and nature of the disease
•    How to manage symptoms
•    Medications
•    Lifestyle and risk factors

Patients are also reviewed by the nurses and various tests may be performed for example, blood pressure, pulse, electrocardiogram (ECG), depending on each individuals need.

The team has close links with a variety of Health Professionals, including GP’s, Practice Nurses, other Community Nurses, Cardiologists and communicate with them as necessary
