Community Matrons
Community Matrons are senior nurses with advanced health assessment/prescribing skills who care for patients at home during an acute episode of their long term condition.
Community Matrons support patients over the age of 18 with long term conditions like chronic obstructive pulmonary disease (COPD), diabetes and heart failure in their home to reduce unncessary hospital admissions and to help patients understand and become experts in managing their symptoms.
The service operates seven days a week and is supported by the Out of Hours District Nursing service over the 24 hour period. Patients have a named Community Matron that they can contact if their condition alters or deteriorates.
The Community Matron Service is based at Stoke and Tunstall Health Centres and is supported by case managers and heatlhcare support workers.
