Specialist Community Respiratory Service

Stoke on Trent Community Health Services have a Specialist Community Respiratory Team comprising of Specialist Nurses, Physiotherapists and Assistant Practitioners.  We are providing specialist support for community practitioners (GPs, Practice Nurses, Community Matrons and District nurses) in the first instance, for COPD management, but with a rolling programme to broaden out to asthma management, bronchiectasis, etc.

This service is for patients registered to a Stoke on Trent GP. The service has a base at Stoke Health Centre.

We offer:

  • Home visiting for acute exacerbation of COPD (step down from UHNS & step up via Single Point of Care)
  • Multi Disciplinary Community clinics (Haywood Hospital and Longton Cottage Hospital in the first instance, to include Cobridge Health Centre when online)  where patients can be reviewed by a Respiratory Consultant / Specialist Nurse and an action plan provided – mostly GPs/ Practice Nurses and Community Matrons utilising this service – offering diagnostic or management support
  • Oxygen assessment – clinic or domiciliary visiting – assessing patient on oxygen therapy, or initiation of oxygen therapy with a diagnosis of COPD
  • Pulmonary  Rehabilitation – 4 locations across Stoke on Trent
  • Supporting staff with education and training to ensure able to confidently carry out and interpret tests

 Age group that COPD effects are 35 years and upwards

Prevention of admission is, in the first instance, supporting step up care in the community, and supporting staff at UHNS in their early discharge.  Long term admission reduction is around providing rehabilitation (evidence supports that this improves the patients self management of their condition, reduces exacerbations, and GP consultations), ensuring accurate diagnosis and appropriate treatment, therefore identifying patients earlier, and hopefully slowing the impact of the disease on the patients life. Ultimately the goals focus on the patient managing their long term condition and maintaining independence.
