Specialist Community Diabetes Service

Stoke on Trent Community Health Services have a Specialist Community Diabetes Team comprising of:

  • Diabetes Specialist Nurses
  • Diabetes Care Technicians
  • Diabetes Consultant
  • Specialist Podiatrist
  • Senior Dietician
  • Clerical support staff

This service is for people with Type 1 or Type 2 diabetes, aged 16 and over who are registered to a Stoke on Trent GP.  The team are based at Stoke Health Centre.

The service operates Multi Disciplinary Clinics (MDT Clinics) to ensure that patients can access specialist services in community settings close to home. By bringing Specialists together in the clinic setting, the patients benefit from quality services, better communication between specialists and improved outcomes for patients.  MDT Clinics operate from 3 main ‘hubs’:

  • Haywood Hospital
  • Bentilee Neighbourhood Centre,
  • Blurton Health Centre (temporary venue whilst work is being carried out at Normacott House, Longton Cottage Hospital).

 Diabetes Specialist Nurse (DSN) clinics continue to be held in Tunstall, Smallthorne, Hanley, Meir, Stoke, and Hanford Health Centres.

Home visiting is available for people who are housebound.  Structured patient education programmes are available for people with Type 1 Diabetes.

The community diabetes team also work with practice nurses and GP’s in a variety of ways to help patients manage acute and long tem complications to avoid hospital admission and improve quality of life.  This is done through informal and formal training, insulin management programmes and mentorship and, drawing up guidelines and protocols which can help to support all health care professionals who are involved in providing diabetes care.
