District Nursing
The District Nursing service provides nursing and end of life care to patients in the community.
District nurses provide nursing care to anyone over the age of 18 who is registered with a Stoke on Trent GP and to adults not registered with a local GP but who reside in Stoke on Trent. Services are provided in the patient's own home, in residential care or in a community clinic.
The District Nursing Team:
- Provide skilled nursing assessments.
- Support, counsel, educate and advise.
- Help prevent complications.
- Access nursing equipment for patients.
- Support patients following discharge from hospital.
- Coordinate care and work with other agencies.
- Provide palliative care including end of life management.
- Assist patients to reach their maximum level of health and independence.
District nurses are assigned to local GP practices. Please contact your GP and they will give you contact details for your District nurse.
